Allies Evaluation Instruments
Instruments and tools were developed or adapted by Allies Against Asthma (Allies) for their cross-site evaluation. A cover page describes each instrument and provides further information to assist in the use or adaptation of these instruments by others.
The Allies Against Asthma program, funded by the Robert Wood Johnson Foundation, supports seven coalitions which aim to develop and sustain community-wide pediatric asthma control systems. Direction and technical assistance for Allies is provided by the National Program Office at the Center for Managing Chronic Disease at the University of Michigan. Allies' evaluation approach was designed collaboratively by leaders from all seven community coalitions, the program's National Advisory Committee members and the Allies National Program Office staff.
