Allies Against Asthma Core Caregiver Survey

The Allies Against Asthma  Core Caregiver Survey can be used to assess individual-level-asthma related outcomes.  This instrument is a compilation of previously existing surveys designed to collect self-report data about asthma management, exposures to community events and programs, and outcomes.  It was designed to measure individual outcomes between baseline and follow-up periods within an intervention and control/comparison group.  It measures the following:

• Asthma Symptoms
• Exposure to Asthma-Related Community Events and Programs
• Parent Asthma Management Strategies
• Hospitalizations and Emergency Department Visits (self-report)
