CMCD is a multidisciplinary group focused on identifying gaps in chronic illness care and how to improve access, efficiency, and outcomes for patients – particularly patients in the most vulnerable communities.

CMCD is located within the Department of Health Behavior and Health Education in the University of Michigan School of Public Health. Affiliated investigators and staff represent units throughout U-M, including the School of Public Health, School of Medicine, College of Engineering, College of Pharmacy, School of Nursing, School of Information, and School of Social Work. We work actively with other universities across the US and internationally, and we have strong ties with the VA Center for Clinical Management Research in the Ann Arbor VA Healthcare system.
